East Tennessee State University Family Medicine was named a 2018 HIMSS Davies Enterprise Award recipient for leveraging the value of health information and technology to improve outcomes for transitional care management and quality scores.
East Tennessee State University Family Medicine created a transitional care management clinic in each of their family medicine clinic locations. Am intraprofessional interdisciplinary care team worked to improve the patient experience by prioritizing follow-up appointments with the help of ADT (Admission, Discharge, Transfer) feeds within the electronic health record. This decreased costs, readmission/admission rates and increased access to care.
After joining an Accountable Care Organization and 13 managed care contracts, East Tennessee State University Family Medicine recognized incorporating process driven by health information and technology were critical for the organization to improve workflows. The organization built custom databases and performed integrity checks that led to improved quality scores.
